INFORMATIONAL SECTION TO READ BEFORE ORDERING

CONFIDENTIALITY STATEMENT:
All counseling services are kept confidential in accordance with State and Federal laws.  By ordering electronically, you are providing LaRose permission to communicate with you using the method you provide in your order (email, phone, and mailing address).

Information related to your order with LaRose will be processed by PayPal and your bank for the purposes of payment.  Such payment processes may inadvertently inform others that you have elected to employ Kurt LaRose, MSW, LCSW for services however the content of sessions, phone calls, emails, etc. etc. will not be disclosed by LaRose to other parties without your written permission.  Confidentiality will not prevent others from seeing that you have placed an order online, for example when you place an order to make a payment to LaRose (in other words, banks and online credit card processing companies will obtain billing and payment information between you and LaRose).

Any other purchases that you make from LaRose, that are not covered by state and federal confidentiality laws (such as brochures, web pages, and inservices) will be kept confidential at your request. 

REFUND POLICY STATEMENT:
Your payment indicates your desire to purchase services or products from LaRose.  This ordering page has been set up solely for your payment convenience and to provide a place where you can make secure online purchases from LaRose.  These payment processing services (PayPal for example) charge fees, which are included in the prices listed here.  However, in the case of an issued refund, your purchase price(s) will be reduced by the transaction fees, and percentage cost fees charged by PayPal and/or the credit card company.  Please know that refund terms for professional services are not usually handled in the same manner as retail goods, thus you may not be able to obtain a refund for therapy sessions, consultation services, etc. etc, even if you are dissatisfied with those verbal exchange types of services.  However, in the case of an order for goods, such as flyers/brochures, if you request a refund for these items, the flyers must be returned prior to a refund being issued.  But again, there will be no refunds offered or provided for professional services that have already been rendered.  In the event you order mental health / consultation / professional development services, please know that a full disclosure and in-take form will be emailed to you for your review and signature.   

CONFIRMATION PROCEDURES:
Your payment indicates that you have read the refund policy and that you are agreeing to utilize one or more of the services being offered by LaRose.  Confirmation documents will be forwarded via email and or by standard US mail to the address you provide.  Please read the confirmation materials, as some of the materials may require your return signature (if so, a postage paid envelope will be included for your convenience).  For in-service and professional development orders, we will contact you to schedule the seminar and to obtain information on how many will be attending; for office appointments we contact you to schedule your first session; flyers and brochures will be mailed within two weeks.

LIMITATIONS TO SERVICE DELIVERY:
Counseling services are only available in Florida, where LaRose is licensed to practice.  It may be possible to order online counseling services, such as chat sessions for example, however you should know that these services are being provided from the state of Florida and under the licensing authority of Florida.  If you are attempting to obtain counseling services via the web please know that you are encouraged to contact a provider within your state first, and/or to contact your state’s mental health licensing board to review their policy and procedures for online services that are in the mental health sector.  Either way, any mental health services that you obtain online should be provided by a licensed mental health professional who meets a national examination (like the ASWB LCSW exam) and who has a current/active/valid state license.  LaRose is licensed via the national ASWB examination in the State of Florida.
